District 14 Membership/Charter Application

Online membership application and PayPal payment here

To join our organization or renew your membership please complete the following steps:

  • Download and print the application here for individual membership.  Charter forms for clubs are here.
  • Complete and sign the application.
  • Mail a copy of the application along with your check/money order (made out to District 14 Membership) to:

Please note that there is a $3.00 service charge for duplicate membership applications if you sign up and forgot your card or receipt. We ask that you allow 21 to 30 days from the date you mailed your application for processing. Until you receive your new card, your copy of this application may be used to sign up and ride in AMA District 14 events for 60 days following the date of application.

If you have not received you D-14 membership card in 30 days contact D-14 Membership with information on where and whom you applied for membership.
